Nomination Form for the Fall 2015 New Faculty Workshop

The deadline for nominations has been extended until Wednesday September 23 at 5:00 pm Eastern Time.

Please enter all requested information. Begin by entering the nominee's information and finish by entering your name, email address, and any additional comments in support of the nomination. A confirmation will be sent to you and the nominee.

Note: If you are a department chair and wish to apply to be a participant in the workshop, please ask your Dean to submit the nomination form using the Dean's name and email address in substitution for Department Chair name and email address on the form.

By nominating the faculty member for the New Faculty Workshop, you are agreeing that your institution will pay for the nominee's travel to and from College Park, MD and the $375 workshop registration fee. The registration fee is payable once the nominee is accepted for the workshop. Limited financial support, primarily for faculty from minority-serving institutions or from departments with severely limited financial resources, is available.
