Continuing Education Units (CEU) Request Form

Certified Workshop leaders can use this form to request an Application for Continuing Education Units for participants in their workshops. When this request is approved by AAPT, the appropriate registration and evaluation forms will be mailed to the workshop leader within 5 working days.

If the request for CEU is denied, the workshop leader will receive an e-mail with reasons why the request was denied, or asking for additional information.
